The role is responsible for responsible for client centred care planning and clinical care management and supporting the Client Care Operations Manager in achieving strategic objectives and growing the clients we care for at HammondCare.This role requires a deep understanding of aged care services, a client-centred approach, and strong communication and coordination skills. You will work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans that align with each client's unique needs, preferences, and goals.# Key Focus Areas:* Assess and understand individual client needs, including clinical needs and preferences to develop tailored care plans that align with service expectations* Coordinate and oversee client centred care planning to ensure services are appropriate, responsive and person-focused* Assist clients in developing and managing a budget that aligns with their funding, care needs, and service priorities.* Manage and monitor the delivery of in-home support services ensuring compliance to Aged Care Quality Standards* Build and maintain strong client relationships by providing ongoing support, communication and guidance throughout their care journey* Collaborate with multidisciplinary teams to document, review and ensure compliance with care plans and all related activities* Engage in ongoing care discussions with clients, carers, and other key stakeholders, facilitating case conferences as needed.* Provide client education using a reablement and health promotion approach, including:* Supporting informed decision-making* Providing independent advice, information, and resources* Assisting with system navigation and service linkage* Identifying and problem-solving issues and risks* Advocate for clients, ensuring their views, rights, and concerns are acknowledged and escalated when necessary.* Assist clients in providing feedback and complaints and take an active role in managing and resolving concerns.* Communicate clearly and intentionally with staff, clients, and families to ensure transparency and engagement.* Manage client care in alignment with HammondCare’s Mission, Policies and Procedures, Strengthened Quality Standards, and the Support at Home Program Manual.* Participate in continuous quality improvement initiatives to enhance care services and client outcomes.# * Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.* ***University-level qualification in a relevant health-related discipline, for example, Bachelor of Social Worker, Physiotherapy or Nursing*** OR ***Certificate IV in Aged Care, Certificate IV in Disability, Certificate IV in Community Services, Diploma of Community Services (Case Management) or Diploma of Ageing Studies and Services**** Experience in aged care, home care, or community care, with a passion for client-focused service delivery.* Strong ability to develop, implement, and review personalised care plans.* Excellent communication and relationship-building skills, with a commitment to advocacy and person-centred care* Knowledge of Aged Care Quality Standards and compliance requirements.* Experience working collaboratively with healthcare professionals, carers, and external providers.* Strong problem-solving, organisational, and coordination skills.* Experienced in operating effectively in a multi-site environment.* A current driver’s licence and safe driving record.
