Responsibilities Conduct comprehensive assessments of clients' physical and functional abilities Develop and deliver individualised or group treatment plans based on clients’ needs, goals, and preferences Implement evidence-based physiotherapy interventions to address mobility issues, pain management, and other age-related conditions Regularly assess and report on clients' physical and psychosocial capabilities to the care management team Maintain precise records, exercise sheets, assessment reports and progress notes that accurately reflect clients’ physical function Make recommendations for exercise programs, mobility equipment, safety at home and refer to other disciplines as required Uphold ethical and professional standards in accordance with regulatory guidelines Collaborate with interdisciplinary teams, including care advisors, occupational therapists, fitness instructors, allied health assistants and other healthcare professionals, to provide holistic care Educate clients and families on preventive measures and exercises to improve and maintain mobility Participate in team meetings, case conferences, and training sessions to contribute to continuous improvement of care delivery Engage with team members, clients and families in a positive and respectful manner, welcoming a culturally diverse team Build relationships with clients and their families, understanding the challenges they face in aged care Qualifications Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration Excellent interpersonal, verbal, and written communication skills Ability to manage all aspects of compliance Demonstrated experience with multidisciplinary team working Effective time management skills to ensure patient intervention, reports and outcomes are achieved in a timely manner and the ability to effectively prioritise and organise own caseload Demonstrated computer literacy Demonstrated knowledge and experience of health, aged and/or disability services Current driver’s licence and a registered and insured motor vehicle What We Can Offer You Paid travel time & km’s reimbursed Not-for-profit tax benefits so you pay less tax Access to professional development and clinical support (up to $2,000 CPD allowance per year) Fitness Passport discounted gym benefits Paid parental leave Long Service leave after 5 years 40+ locations across NSW and QLD if you choose to move A values-based organisation that welcomes applicants from all faiths and backgrounds Catholic Healthcare acknowledges the Traditional Owners and Custodians of the lands on which we live, work and dream.
