About the facility Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. About the role The Planning and Referral Team (PaRT) are experienced health clinicians who work with the treating teams, patients, families and services in planning an appropriate, timely and safe discharge. Responsibilities include: Facilitate and coordinate patient transfers to bed‑based services and coordinate referrals to community supports. Assist with discharge planning in collaboration with the medical and other allied health teams. Make required referrals, share information and communicate with all stakeholders.
