The Opportunity The Mental Health Clinician will be a key clinical support within the Hospital in the Home (HITH) program as part of a rotating roster. This position is central to delivering comprehensive care, evidence informed care within the HITH model. HITH is an acute service providing early intervention to young people with severe and complex mental illness within the home environment. Operating seven days a week, the multidisciplinary HITH team delivers accessible, collaborative, and coordinated mental health care to young people aged 12 to 25 years and their families/carers across Melbourne’s northwest and western suburbs. Our service is made up of compassionate, curious, and dedicated professionals. As we continue to grow, we are seeking an experienced, kind and collaborative Mental Health Clinician to support the delivery of comprehensive acute and complex mental health care to young people and their families of the Hospital in the Home program. You will demonstrate high standards of clinical care that are responsive to each young person’s developmental stage and presenting issues, and informed by existing evidence and guidelines. The role offers you opportunities for providing key discipline specific individual, family and systems interventions aimed to support and facilitate recovery and well‑being. We deliver individually formulated care plans that comprise of psychopharmacology, therapeutic interventions (CBT plus other targeted interventions), family inclusive practices, vocational and educational support, intensive outreach, and crisis support. What You'll Do Complete mental health assessments, including formulation of care plans, crisis management plans, outcome measures, progress notes, discharge summaries, and all other required documentation. Undertake mental health interventions, crisis response, and family and systems based work within the client and carer’s context. Ensure care is guided by evidence based, culturally safe, and trauma‑informed principles that incorporate the diverse needs of young people, their families, and support systems, to facilitate recovery and promote wellbeing. Demonstrate the capacity to provide, or develop skills in, integrated assessment, treatment, and care for young people with dual diagnoses. Provide responsive, strengths‑based, and youth‑focused care and advocacy for all young people and their families, including those from culturally and linguistically diverse backgrounds and the LGBTQIA+ community. Contribute to a culture of compassionate teamwork, learning, and continuous improvement. About You You will be experienced in, and passionate about, providing acute, developmentally appropriate, and evidence-based mental health care to young people and their families in the community, including those requiring complex support through the Hospital in the Home program. ✔️ Eligible disciplines: RPN Grade 3 / Occupational Therapist, Social Worker and Psychologist Grade 2 ✔️ Relevant registration and qualifications (see position description for details) ✔️ A minimum of 12‑24 month’s experience in tertiary clinical practice within the mental health field, including young people experiencing mental health disorders ✔️ Ability to develop, implement, and evaluate client‑centred individual service plans, risk assessments and diagnostic formulations ✔️ Clinical experience and skills in engaging and working collaboratively with families in a mental health setting ✔️ Demonstrated interpersonal and communication skills (written and verbal) as well as abilities in problem solving and negotiation ✔️ Motivated by youth-focused, inclusive, and recovery-oriented care Ready to Make a Difference?
