This position offers valuable experience across a range of acute settings, rotating every three months through our Medical, Surgical and Rehabilitation units. In this role, you’ll independently prioritise referrals, complete timely assessments, and provide recommendations that support safe and effective discharge planning. Your work may include functional assessments, home visits, equipment prescription, cognitive screening, and the completion of guardianship or NDIS applications when required. You’ll also contribute actively to multidisciplinary team meetings and collaborate closely with colleagues to achieve the best outcomes for patients transitioning back into the community.
