ABOUT THE ROLE We are looking for aCare Partner, Registered Nurse who is a passionate health and aged care professional looking for a new opportunity. As Care Partner you will be an integral point of contact for workers and clients to provide support for the broader community team. Join us on a permanent full-time capacity at Right at Home Southern NSW, located in Wagga Wagga. To be successful you must hold: Formal tertiary qualification eg. Bachelor of Nursing or equivalent Current AHPRA registration as a Registered Nurse (Mandatory) National criminal history/police check Valid First Aid and CPR certificate Valid Working With Children's Check Card Experience: Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards – Essential Previous experience in Aged Care or related discipline – Desirable Easily adaptable to new software platforms As aCare Partner you will be responsible for: Building strong and meaningful connections with clients and their family Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home Interacting and providing professional health and clinical care advice and direction to a team of home care workers Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction Ensuring all practices are consistent within policy and legislative frameworks Maintaining up to date client records and information ABOUT US Right at Home is a leading provider of in-home care and assistance, committed to improving the quality of life for our clients we serve.
