About the Role Aboriginality is considered an essential criterion of this position under Section 50 (D) of the Equal Opportunity Act 1984. Based in Albany, Western Australia, reporting to the Projects & Services Manager, the Aboriginal Youth Empowerment Program AYEP aims to provide culturally safe, trauma informed, and individualised case management, mentoring and goal setting, for disengaged or at‑risk Aboriginal youth, and to empower participants to achieve self‑determination and positive life outcomes. The Aboriginal Youth Worker will deliver a strengths‑based, community mandated program focussed on personal growth, emotional resilience health, wellbeing, and employment outcomes with a strong focus on cultural identity, community connection and sustainable impact. The program integrates group workshops, on‑Country activities, career readiness, education support, and wellbeing initiatives, all imbedded in Aboriginal culture, and guided by Elders and cultural mentors. About You You are a motivated and committed person that essentially has: Formal qualifications or equivalent experience in social and community services. Experience in similar case management, assisting young people. And desirably has: Demonstrated experience in the development of case plans structured to achieve identified goals. Demonstrated use of organisational processes and systems to ensure that all documentation requirements are completed in a professional and timely manner. Demonstrated awareness and knowledge of working with the target cohorts and broad knowledge of issues affecting people from these communities. Demonstrated ability to liaise, consult and form cooperative and purposeful relationships with staff members, community, government agencies, service providers, ACCOs and other stakeholders. Highly developed interpersonal, communication and negotiation skills and the capacity to build and maintain relationships with a diverse range of people. Mandatory You must have: A current WA Driver’s licence You must provide before you start: A Prior Offender History Check no older than three months. A Working with Children Check. Key Responsibilities Promote AYEP to service providers, stakeholders and community across the Central Great Southern Implement and deliver intake referral processes with community Provide initial and ongoing assessment of client needs, including risk assessment and case planning Provide proactive, persistent, reliable, practical and comprehensive support in a manner that is respectful of the dignity and privacy of individuals Contribute to identification of issues and setting of goals and tasks that are mutually agreed with and achievable for clients Facilitate multi‑agency case management meetings and cooperate with other services as necessary and appropriate to support positive outcomes for clients Provide warm referrals of clients to external service providers to access specialist services Provide warm referrals of clients to internal SAC programs Employ a culturally safe, trauma informed Ensuring that practices are compliant with standards of care and sector standards Provide respectful and flexible services that are appropriate to the range of cohorts accessing AYEP Adopt policies and practices that enhance access and are culturally safe and appropriate for the target cohort of clients Accurately maintain client data in accordance with relevant requirements Provide other reports as required and as negotiated with funding bodies and referral agencies Ensure information and support is provided to program clients in ways that they can understand.
