Access Support Coordination – Rockhampton QLD As a Support Coordinator at Access Support Coordination, you will guide participants in implementing their NDIS plans, ensuring they understand their options to make informed choices. You will coordinate the implementation of these supports, optimising capacity and funding management. Key Responsibilities Build a connected life for people outside of funded support. Guide participants as they implement their NDIS plans, helping them understand options to make informed choices. Coordinate the implementation of supports, ensuring capacity and funding management are optimised. Support participants to collect key evidence that supports their changing needs, and submit funding requests when required. Explore meaningful opportunities in recreation, employment, housing, and more to enrich their lives. Ensure mainstream services step up and fulfill their commitments. Provide regular, insightful reports to the NDIA and participants on progress and achievements. Achieve all relevant KPIs, including billable hours. About You Qualifications in social work, disability, mental health, or a related field. Experience in support coordination, case management, or similar. In-depth knowledge of the NDIS and its range of services. Confident in meeting KPIs including billable hours (highly regarded). Outstanding communication skills for diverse participants, providers, and stakeholders. A natural problem-solver, multi-tasker, and decision-maker with keen attention to detail.
