The interventions can range from advice and recommendations to prescription of equipment and modifications. The role also provides coverage to residents and acute inpatients in Multi-Purpose Services in smaller towns around Tamworth. Orientation and support will be provided by your manager and other member of the wider team. Where you'll be working This position is based at Tamworth Community Health Centre in the Integrated Aged Care Team, which is made up of includes the Transitional Aged Care Program (nursing, OT, PT, AHA, Domestic), Clinical Nurse Consultants specialising in Dementia / Delirium, Psychogeriatrics, Continence and Women’s Health, End of Life Clinical Nurse Educator, and immunisation programs. This role works closely with Occupational Therapists from the Tamworth Community Health NECACS team and is clinically supported by a Level 4 OT in that team.
