Support at Home Coordinator

Aman Care is looking for a Support at Home Coordinator who genuinely cares about helping older people live well at home. This role is ideal for someone who is confident working in the aged-care space, enjoys supporting a team, and is comfortable balancing hands‑on work with leadership and oversight. You’ll be a key point of contact for staff, clients, and families, helping people navigate what can sometimes be a challenging stage of life with dignity and clarity. What the Role Involves In this position, you’ll have the chance to make a real difference each day by: Building and maintaining a steady client base each month, including outreach, relationship management, and ongoing engagement with participants and families. Leading the Support at Home program and keeping things running smoothly across day‑to‑day operations. Supporting Care Partners with clinical guidance, advice, and on‑the‑ground problem‑solving. Carrying out (or overseeing) clinical and functional assessments, ensuring everything is documented properly under relevant guidelines. Managing more complex cases; including restorative, chronic, or end‑of‑life support, and stepping in where extra oversight is required. Working closely with families, GPs, allied health, community services, and team members to make sure support is coordinated and timely. Reviewing individual care plans so they stay relevant as clients’ needs change. Monitoring participant budgets and ensuring funding is used appropriately. Helping to build a positive team culture where people feel supported and confident in their work. Championing culturally aware, respectful care for clients from all backgrounds. We’d love to hear from you if you: Have experience in aged care, home care, community services, or a related field. Hold a nursing qualification and current APHRA registration. Have confidence in building rapport and maintaining client relationships. Understand the Support at Home, CHSP or Home Care Package environment. Enjoy supporting others and have experience guiding or mentoring staff. Are comfortable working with clients who have complex needs. Communicate clearly and respectfully with a wide range of people. Are organised, practical, and able to juggle competing priorities. Are culturally sensitive, patient, and committed to person‑centred care.
