Kinship and Targeted Care Packages Case Manager BDAC is an ACCO (Aboriginal Community Controlled Organisation) registered as a member under the umbrella of VACCHO (Victorian Aboriginal Community Controlled Health Organisation) and represented nationally through NACCHO (National Aboriginal Community Controlled Health Organisation). BDAC was founded to represent and provide services to Aboriginal and Torres Strait Islander people living on Djaara Country. BDAC has a responsibility to ensure growth of services, development of our Aboriginal and Torres Strait Islander community, better and improved health outcomes for our people, improved quality of life and be a lead agency in providing self-determination employment and career pathways for Aboriginal people. What We Are Offering Part time 30.4 hours per week Fixed term 12 months SCHADS 4 - $44.48 per hour Access to Salary Packaging 12% Superannuation Supportive work environment Ongoing training and development opportunities About the Position The TCP Worker will work alongside the Case Contracting and Cultural Programs Team to develop and submit Targeted Care Package (TCP) proposals in conjunction with other agencies that are involved to ensure the package will meet the full needs of the child or young person involved. The TCP Worker will also work with both the child and the parents/carer or legal guardian that is caring for the individual. Key Responsibilities Develop and assist eligible clients with the design of their TCP including consultation with the individual, extended family and carer for persons in care. Ensure that other external agencies are involved in the design of the TCP. Participate on TCP panels and ensure that each TCP meets the needs of the individual. Advocate on behalf of the client if the TCP does not have enough support for the individual. Provide appropriate case management with the client and link with BDAC programs and/or any other appropriate referral agencies. Ensure the individual and any other relevant people are communicated to regularly on their progress of TCP’s. Review each TCP at least once every six months or as required to ensure it still meets the needs of the individual. Provide support for the parents/carers to ensure they have the relevant support networks in place. Other duties as requested by your team leader. About You Knowledge of child and adolescent development and strategies for working with vulnerable children, young people and their families with an understanding of children with complex special needs, understanding Trauma. Experience and skill in rapid engagement and goal setting. Experience in development and review of targeted care packages. Excellent communication and interpersonal skills. Understanding of Aboriginal culture. Knowledge of the Aboriginal Child Placement Principle. Demonstrated ability to sensitively work with the birth family, extended family and foster carers during the placement process. Ability to effectively and efficiently plan and organise own workload. An ability to manage stressful situations. Case Management, Data collection and record keeping skills. The ability to work as part of a team and a strong commitment to collaborative working relationships. Demonstrated negotiation and advocacy skills. Understanding of support services available to child, family and carer. Preferred / Desired Education, Training and/or Competencies Formal Qualification in a Social Welfare/Disability discipline or equivalent Beginning Practice (DHHS) Experience in inter-agency collaboration.
