About The Role An exciting opportunity exists for a Health Clinician with clinical leadership skills and the ability to work collaboratively within a multidisciplinary team of dedicated clinicians. You will need to be able to plan, deliver and coordinate patient care, working collaboratively to provide streamlined efficient and effective care to avoid duplication of service. You would be working within a small virtual team in the Western Diabetes Hub with five clinicians and administrative support, while working closely with patients/clients. Location is negotiable, with typical hours 08:00 – 16:30 or similar. The successful applicant will be provided with a comprehensive orientation and supervision for obtaining credentialing hours (if not credentialed) during employment. Participate in credentialing processes and undergo orientation and supervision as required.
