About the Role An experienced Registered Nurse is needed to join the Residential Services team as a Clinical Care Coordinator. The role involves delivering safe, evidence‑based and person‑centred clinical care by developing and reviewing care plans, identifying and responding to changes in residents’ health, and ensuring effective communication with families, staff and allied health professionals. Culturally safe, trauma‑aware and healing‑informed practices are applied to support residents’ wellbeing and empower human‑rights‑based decision‑making. Key Responsibilities Deliver safe, person‑centred and trauma‑aware clinical care aligned with Aged Care Quality Standards and AHPRA guidelines. Develop, implement and review care plans, ensuring residents’ clinical and psychosocial needs are met, including advanced and palliative care. Monitor resident health, recognise and respond to deterioration and support best‑practice interventions that promote independence and wellbeing. Coordinate with medical, allied health and lifestyle teams to ensure smooth care transitions and holistic outcomes. Review clinical data and documentation to identify trends, support AN‑ACC funding accuracy, and drive continuous improvement in care quality. Respond to feedback and clinical concerns promptly, ensuring effective communication and resolution. Qualifications Current AHPRA registration as a Registered Nurse (Division 1) – no restrictions. Minimum 3 years’ experience in aged‑care nursing, with a sound understanding of the ageing process. Ability to assess training needs, design sessions, deliver staff training and organise external education/training. Proven ability to assess, plan and evaluate resident care and clinical risk. Strong leadership and mentoring skills, with the ability to build a positive team culture. Sound knowledge of Aged Care Quality Standards, SIRS, and AN‑ACC requirements. Experience in culturally diverse environments; Greek language skills are an advantage. Why choose Fronditha Care?
