Main duties of the job To provide comprehensive, holistic therapy assessment and rehabilitation to patients within their home environment, maximising independence within a 4 -week pathway. To hold clinical responsibility for patients within the service and Therapy Practitioners within their team. To prioritise workload including that of the Therapy Assistants within the team. To work proactively, in collaboration with the wider multidisciplinary team to ensure patient flow and safety. To assist in the development of the Integrated Care Workers and Therapy Practitioners to support the clinical delivery of the service. You will need to hold a full UK driving licence* and have use of a vehicle. Person Specification Education / Qualifications Degree or equivalent eg.
