We are looking for a new Practice Facilitator based in Whangārei to support our primary care general practices.

This is a Relationship Role

Are you a relationship-driven person and able to or experienced in building and maintaining strong positive relationships with stakeholders? Our role is centered around relationships, regular interaction, and supporting our primary and community care health professionals in general practice. This is an exciting opportunity to either develop or expand your knowledge of primary and community care, and to support the future improvements in primary care service delivery and access to community healthcare services for our Northland population.

In this role, you will engage with General Practices and guide an improved understanding and application of equity across primary health care services. Underpinned by the National Pae Ora Healthy futures framework, our Mahitahi Papa Tikanga framework, and Equity for Whānau agreement, this role will support capability for primary healthcare professionals in embracing an equity for whānau focus and approach.

About Mahitahi Hauora

Mahitahi Hauora is a primary health entity underpinned by Te Tiriti o Waitangi. Our Kaupapa includes ensuring whānau and communities are able to achieve self-determined wellbeing, access to the services they determine they need, and to live a long and healthy life. We work collaboratively with community, primary healthcare providers, and key partners to support general practices, Māori health providers, and social sector agencies, in achieving better health outcomes for our Tai Tokerau population.

The Position

This position is based in Whangārei and supports a number of practices and providers across the greater Whangārei region. You will work alongside your Practice Facilitator colleagues in supporting practices and providers across primary healthcare programmes, improvement methodologies, and equity strategies. This is a varied role that provides an opportunity for growth in skill set, relationship management, and quality and service improvement initiatives.

You will need to be passionate and enthusiastic in your approach, understand the primary healthcare and community environment, and be prepared to be flexible and innovative.

This role would be ideally suited to someone who has a current or previous background in health, and who may be looking for something a little different or moving away from direct clinical work but still wants to be involved in the health or community sector. We are looking for the following experience and skills to match, however, all applications will be considered relative to below:

  1. A minimum of 3-5 years’ experience in primary/community healthcare
  2. A relevant tertiary level qualification or equivalent training/experience
  3. Your experience in developing and managing relationships with health professionals
  4. An intermediate to advanced level of computer literacy across MS Office 365, Teams
  5. Experience in implementing change in an organization
  6. Evidence of a confident communicator both spoken and written
  7. An understanding of and commitment to Te Tiriti o Waitangi
  8. Your understanding and knowledge of equitable health outcomes
  9. A current full NZ driver's licence

You must be a NZ Citizen or Permanent resident. We may consider those with a current open NZ work visa that is not tied to an accredited employer.

While not essential, an understanding or experience in change management strategies and improvement methodology would be desirable, and some experience in primary care practice management systems and analytics would be useful.
