We are currently seeking to expand our fabulous team with another proactive, vibrant and motivated Support CoordinatorAbout Us:At My Plan, My Way we pride ourselves on the person centred and individualised supports we provide to NDIS eligible participants in Toowoomba and surrounds.We are a boutique team of coordinators who have been navigating the NDIS since its rollout. Our personable crew bring a wealth of knowledge and life experience that allows us to work closely with participants and their families to support them to achieve their goals and desirable outcomes.What we're offering:Contract position with the possibility of extensionPart Time hours, negotiableFlexible working hours in the dayGrowth OpportunitiesPortable Long Service LeaveWork / Life BalanceAbout the Role:As a Level 2 Support Coordinator you will be responsible for coordinating supports in line with Participant's NDIS plans which support them to achieve their goals by:Locate, coordinate and facilitate a range of supports as per identified needsConnect Participants with mainstream supportsTrack plan usage and ensure supports fit within budget constraintsEnsure supports are working toward Participant goalsWork collaboratively with Participants, informal and formal supports, providers and other stakeholdersAll training providedExperience/ Qualification/ Requirements:Cert III in Disability or Community Services, or similar preferredExperience working with people or lived experience with disability essentialNational Police Check and NDIS worker screening or willing to obtainSound written and verbal communicationAttention to detailReliable vehicle and driver's licenceApplications without a cover letter and up to date resume will not be considered.
