Care Manager/Case ManagerOur client is a leading provider of aged care & disability support services, dedicated to helping individuals maintain their independence & live comfortably in their own homes. Location: South Western Sydney - in office 5 days a weekSalary Range: $80,000 - $100,000 incl.
Super (depending on experience)Hours: Monday – Friday, 38 hours per weekWhat they offer: Training & career developmentOnsite parkingMonthly eventsExceptional office cultureWeekly Friday afternoon drinks & nibbles at Head OfficeHealthy snacks & refreshments availablePrimary Purpose of the Role:Care Managers support & empower clients by building strong relationships, assisting them to maintain independence at home & ensuring their care and support needs are met through effective case management.
This includes overseeing the intake, planning & ongoing review of Home Care Package (HCP) & National Disability Insurance Scheme (NDIS) clients, while ensuring compliance with legal & contractual requirements.
Key Accountabilities: Meet, engage & build relationships with clients to develop, refine & achieve their goalsProvide information & support to clients & their families/carers to monitor the use of their fundsAssist with all end-to-end HCP & NDIS client intake, including but not limited to client enquiries; client visits & onboardingAccurate case noting & client reviews that reflect forward planning in line with the participant's NDIS planUndertake initial assessments & assist in annual reviews conducted for HCP clientsBuild & maintain strong relationships with all stakeholders including government & non-government bodies, service providers, allied health professionals & practitionersRead, understand & maximise the value for money of NDIS plansWork collaboratively with the rostering & clinical department to ensure clients receive care & support in line with care plans & funded level of packageAbout You: Demonstrated case management experienceProven knowledge of the Aged Care Home Care Package FundingExceptional customer service, negotiation & influencing skillsAbility to connect with multiple stakeholdersStrong interpersonal skills & high levels of resilienceDriver's license & access to own vehicleOutstanding verbal & written communication skillsIf you have relevant skills & experience, please submit your latest CV.
