The Northern Adelaide Local Health Network (NALHN) is eager to recruit an Allied Health Professional with an interest in learning and being supported in providing behaviour change therapy to work with clients managing conditions like diabetes, respiratory disease and heart disease.
About the Role: The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.
The Allied Health Intermediate Care Service (ICS) comprises of Dietetics, Exercise Physiology, Occupational Therapy, Podiatry, Physiotherapy, Social Work and interdisciplinary complex self-management support services (CHAMP), operating as an intermediate service between hospital admission and community-based supports.
Our team works collaboratively and in alignment with other services within NALHN Clinical Divisions as well as local Aboriginal Health Services, with a focus on optimising functional and health outcomes and reducing hospital admissions for clients with Diabetes, Chronic Cardiac and Respiratory Conditions and vulnerable people with other chronic conditions.

Working in CHAMP as a Chronic Disease Self-Management Practitioner, you'll play a key role in supporting clients on their journey toward better health by: collaborating with clients to set realistic goals and care plans to achieve lasting lifestyle changes providing ongoing motivation, education and support to encourage self-management strategies working within an interdisciplinary team of healthcare professionals to deliver comprehensive care continuously evaluating and adjusting therapy techniques to ensure the best possible outcomes for each client To excel in this role you must: possess knowledge and experience working with clients with chronic conditions and an understanding of the daily impact on clients lives (particularly diabetes, respiratory disease and heart disease) have a passion for helping others achieve their health and wellness goals possess strong communication skills and the ability to build trust and rapport with clients have desirable experience in behaviour change, health coaching or chronic disease management and experience with evidence-based behaviour change frameworks (eg motivational interviewing, CBT etc) have the ability to work independently as well as part of a multidisciplinary team show empathy and patience with a client-centred approach to care To be eligible for this position, you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically: Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Dietitians Association of Australia (DAA).
Appropriate Degree or equivalent qualification, usually in human movements, inclusive of the exercise physiology elective and eligible for accreditation from Exercise Sports Science Australia (ESSA).
Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
Appropriate Degree or equivalent qualification which entitles registration with the Podiatry Board of Australia.
Appropriate Degree or equivalent qualification recognised by the PsyBA and general registration with PsyBA.
Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
A certificate in the Flinders Model of Chronic Condition Management (or willingness to achieve this with in-house training) is highly desirable.
