About the role
Permanent Part time opportunity to provide high level clinical pharmacy services and leadership to aid in the care of older persons in the community including those residing in Residential Aged Care Facilities (RACF).
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide and be individually accountable for the delivery of high-level clinical pharmacy services specialising in aged care in accordance with prescribed professional and ethical standards.
This includes but is not limited to medication history taking, medication reconciliation, conducting comprehensive medication reviews and Therapeutic Drug Monitoring (TDM) with minimal direct supervision.
Provide high level advice and assistance to line management, relevant service managers and other members of the multidisciplinary team as required regarding Pharmacy service delivery to diverse stakeholders.
Monitor and report on clinical practices and outcomes within clinical service area.
Provide education and clinical practice supervision, education and mentoring to less experienced Pharmacists including undergraduate and post-graduate students, Intern and HP3 level Pharmacists.
