929300 Position Profile: The Clinical Nurse Specialist – Community Health, Health Navigator is responsible for the coordination of health care for vulnerable children and families within the community setting, collaborating and advocating with key stakeholders within and outside the region.
Successful candidates will collect data and prepare reports for specific child health projects, and act as a role model and consultant to nurses and other health service providers providing knowledge, clinical expertise and best practice within the area of responsibility.
The successful candidate will join the recently awarded WA Health Excellence Award for Person centred care Health Navigator Pilot Program team.
Your key responsibilities will include:
Provide clinical leadership and consultancy to the multidisciplinary health team in the areas of specialty, coordinating care for vulnerable children, families and communities within the health service and liaison and advocacy with external stakeholders. Develop and manage systems for the Health Region to ensure children identified at risk/vulnerable are appropriately cared for in the community setting or supported to access specialist care where indicated. Actively engage with a range of stakeholders within the region and external to the region on the management of children identified as vulnerable in the community setting. Promote and facilitate a multidisciplinary team approach to decision making. Develop innovative techniques for complex problem solving for coordination of care for vulnerable children in the community setting. The Child and Adolescent Health Service (CAHS) is proud to be the leading service provider for paediatric healthcare in Western Australia, as the State's only dedicated health service for infants, children and young people.
CAHS is made up of four service areas: Neonatology, Community Health, Child and Adolescent Mental Health Services (CAMHS), and Perth Children's Hospital (PCH).
Consisting of over 6,500 employees, our services are delivered at PCH and King Edward Memorial Hospital (KEMH), as well as across a network of more than 160 community clinics across the metropolitan area.
