The Role
As a UHM Case Manager, you will play a key role in providing culturally appropriate case management for unaccompanied humanitarian minors (UHM) in the Toowoomba region. You will work to promote the well-being, independence, and community connection of young people in kinship care and out-of-home care arrangements, helping them develop life skills and prepare for adulthood.
This is a fixed-term position for 12 months , ideal for someone passionate about supporting young people and working in a trauma-informed environment.
What You'll Be Doing:
Deliver culturally competent case management and conduct psychosocial needs assessments for unaccompanied minors. Develop and implement case management plans focused on young people's needs, aspirations, and mental health. Provide emotional support, promote social connections, and offer crisis intervention when necessary. Liaise with community and government agencies to establish service pathways for young people and advocate on their behalf. Prepare case plans, write case notes, and complete reports within strict timeframes. Supervise Residential/Youth Support Workers and BiCultural Workers (as required), ensuring high-quality, culturally appropriate care. Participate in an after-hours emergency roster and provide out-of-hours support when required. Qualifications and Skills Required:
Tertiary qualifications in Social Work, Psychology, Behavioural Sciences, Occupational Therapy, Human Services, or a related field. A valid Queensland Driver's Licence. Possession of, or willingness to obtain, a Blue Card (Working with Children Check) and National Police Clearance. Proven psychosocial assessment and case management skills, with the ability to design culturally appropriate care plans. Experience working with unaccompanied minors and an understanding of refugee and asylum seeker issues. Strong communication and advocacy skills, with the ability to work collaboratively in a team environment. Experience supervising or line-managing staff. Experience working with young people in out-of-home care. Accreditation in Therapeutic Crisis Intervention. Location: This position is based in Toowoomba.
