The Medicare Mental Health Kids Hub The Medicare Mental Health Kids Hub is a first‑of‑its‑kind service in WA This dynamic one‑stop hub is designed to revolutionise the way we support the mental health and wellbeing of children aged 0‑12 in the City of Swan. Offering early intervention and comprehensive care through a multidisciplinary approach, the Kids Hub is here to make a real difference for kids facing mild to moderate mental health challenges. The Kids Hub is a free service with a no‑wrong‑door approach, providing step‑up, step‑down support for children and their families for as long as they require. The Kids Hub is an initiative of the Mental Health Commission and is run by Parkerville in a consortium of partners consisting of Koya, Lifespan Psychology, Mental Illness Fellowship of WA (MIFWA) and Pregnancy to Parenthood (P2P). About the Role The Child and Family Care Navigator plays a pivotal role in the exciting, multidisciplinary team at the Head to Health Kids Hub. As the first point of contact for children and families, they are the heart of the Hub, providing essential guidance and support as families navigate mental health and social systems. Working closely with children and families, the Child and Family Care Navigator helps identify how the Kids Hub can best support them, ensuring their journey is smooth and tailored to their needs. Accompanying families across all three pillars – Community Development, Family Support , and Specialist Therapeutic Support – the Child and Family Care Navigator uses a step‑up, step‑down model to provide flexible, comprehensive support at every stage. This service provides a safe and welcoming space for all children across the City of Swan. With operating hours from 8 AM to 7 PM Monday to Friday, the role will involve a rotating 3‑weekly roster between the hours of 9 AM and 6 PM (up to 7.6 hours per day) on a Permanent Full‑Time basis at our Midland Location . The role entails a combination of community‑based outreach, in‑home and office‑based models of working. This is a unique opportunity to make a real difference in the lives of local children. Responsibilities Build rapport and trusting relationships with the child and family, and extended family as required. Assist families to navigate complex systems, particularly mental health and local services. Maintain proactive contact with children and families throughout involvement with the Kids Hub on an as‑needed basis. Provide assessments and case management for children accessing the Kids Hub, ensuring coordinated and personalised support. Advocate and promote wishes, thoughts and opinions of children and families, providing choice of interventions and services and provision of referrals as required. Facilitate engaging workshops designed to support and enhance the mental health and wellbeing of children. Provide ongoing liaison, negotiation, consultation and relationship building with Multi‑Disciplinary Team members and other key stakeholders. Ensure support to children and families is provided in a culturally appropriate way. Participate in multidisciplinary team case review meetings, facilitate case tracking processes and development of Individual Care Plans. Assist the Community and Partnership Manager with community education, professional training, presentations and reporting on statistical data. Maintain effective contact records for each client and participate in the evaluation and development of the Kids Hub model. Ensure the safety, health, security and well‑being of children within your care. Other Adhere to all Parkerville Policies and Procedures including the Head to Health operating procedures. Commitment to and participation in Parkerville’s System Improvements. Understand, support and promote the Mission and Values of Parkerville Children and Youth Care. The successful candidates for the positions will hold the following Mandatory Compliance A Valid WA Driver’s License. Working With Children Card or be willing to apply for one. Department of Communities Check. National Police Clearance. Proof of Working Rights. Essential Skills and Experience Minimum Certificate 3 in Community Services or a relevant discipline or extensive experience working within a community setting. Demonstrated experience and knowledge of trauma informed practice in relation to children and families. Demonstrated empathy and sensitivity working with vulnerable children and families. Ability to form trusting relationships with a diverse range of stakeholders from within a child’s ecological system. Demonstrated experience and knowledge of conducting intake assessments and providing warm referrals. Demonstrated ability to advocate amongst professional peers on behalf of children and young people. Experience in providing supportive and psychoeducation‑based interventions. Understanding and knowledge of referral pathways for children and their families within the mental health system. Experience working with people from culturally diverse backgrounds. Well‑developed interpersonal, written and time management skills, with the proven ability to work within a team environment and coordinate others. Ability to remain flexible to accommodate the needs of children and families and service requirements, inclusive of after‑hours work and the responsibility of children as required. Ability to communicate well to a diverse range of people including children, their families, communities, and other stakeholders involved in the care of the child. Skills in report writing. Experience and/or training as a Peer Worker or Support Worker. Experience facilitating group programs with parents and/or children. Knowledge and experience of responding to people in crisis.
