About the facility Our major sites include University Hospital Geelong in Geelong's CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. The position will be based in the Aboriginal Health Unit, located with University Hospital. The team provide support and advocacy for Aboriginal and Torres Strait Islander inpatients and their families. About the role The Aboriginal Mental Health Liaison Officer (AMHLO) is responsible for supporting Aboriginal and Torres Strait Islander patients with health related issues, accessing Mental Health Drug & Alcohol services at Barwon Health. Responsibilities Provide the client with cultural and social support. Attend appointments or medical reviews and assist patients to understand medical information and services. Through consultation assist other health professionals to respond in culturally appropriate ways. Advocate for patients where required. Provide support to the client's family. Liaise and work effectively with community-controlled services to facilitate care planning and continuity of care. Support discharge planning and processes.
