About the role: Are you passionate about making a difference in the lives of seniors? As a Navigator in our Care Finder Program, you'll play a crucial role in helping seniors who need intensive support to understand and access aged care services. You'll also connect them with other essential community supports. In this role, you'll provide localized, face‑to‑face, specialised assistance through a person‑centred approach that honours each individual's unique needs, preferences, values, life experiences, and choices. Person-Centered Approach:Deliver support that respects and values each person's individuality. Community Connection:Help seniors connect with vital community supports and services. What you will do: Working face-to-face with clients to conduct intakes, assess needs, create individual support plans, etc. Assisting clients in understanding and accessing aged care supports, including contacting My Aged Care and supporting through assessment if required Supporting clients to understand their service provider options and make informed decisions based on their unique needs and experience Conducting follow-up check-ins with clients to determine satisfaction with service providers and support Seeking feedback from clients on their experience within the Care Finder program Leading community information sessions on aged care services and the Care Finder program About you: Cert IV or higher in Community Services or relevant industry experience Strong interpersonal and communication skills Local connections with community groups, seniors, service providers, and other relevant stakeholders Experience and detailed understanding of My Aged Care, aged care services, community supports, and eligibility requirements A commitment to delivering a person-centred and respectful approach that fosters consumer choice Ability to communicate effectively with a broad range of people A commitment to supporting the needs of people with diverse backgrounds and life experiences Strong problem‑solving skills Administrative skills and experience with systems, databases, and portals Why work with us?
