The Diabetes CNC will act as a resource person, for the whole hospital, promoting best practice in the provision of patient care through clinical initiatives and service innovation. The Diabetes CNC will demonstrate superior clinical expertise together with outstanding interpersonal negotiation and communication skills to assist with the achievement of patient‑centred goals and the goals of the designated department and organisation. Advanced assessment, problem solving, critical thinking and organisation skills Excellent communication and interpersonal skills High level of reliability and professional conduct Experience in Insulin dose titration in the Inpatient and Ambulatory Care setting in patients undergoing high dose glucocorticoid therapy, TPN and PEG feeds. Experience in Continuous Subcutaneous Insulin Infusions (Insulin Pump Therapy) initiation and management Experience in Continuous Glucose Monitoring Systems, initiation, data upload and management. Experience with Bolus Advisor Meters Experience in using diabetes management technology and associated computer programs. Experience with working in the inpatient and outpatient clinic settings. Experience with the area of research and quality improvement Experience in review and development of policies and procedures. Key Accountabilities Ensure the delivery of high standard of evidence‑based nursing care to patients. Conduct comprehensive screening, assessment, planning, implementation and evaluation. Establish and maintain effective communication with other health professionals and support staff, patients’ families, the community and external care providers. Collaborate with other health professionals to ensure the patient’s safe and effective transition across the care continuum and facilitate access and referrals to appropriate health resources and services. Lead and facilitate the development of systems and processes to support the planning and coordination of treatment, education and support needs for patients with cancer, and their families across the disease trajectory. Support the ongoing development of the clinical services model and provide support and information to nurses in the preceptorship program allocated to the clinical service. Actively engage in the preceptorship program, ensuring succession planning for the role. Contribute to the development of local, national and international evidence‑based care guidelines, policies and procedures and patient resources. Support and promote excellence and autonomy in clinical practice and patient care. Patients and their families experience timely, relevant and personalised care. Perform nursing interventions and procedures in accordance with Peter Mac policies and procedures. Coordinate the assessment, development, implementation and monitoring of a comprehensive plan of care from admission to discharge and in the outpatient setting in conjunction with the multi‑disciplinary team. Supports and promotes excellence and autonomy in clinical practice and patient care. Identify, manage and action risk and actively work towards implementing risk reduction strategies. Contribute to the development of an effective discharge plan and provide appropriate patient education and referral to appropriate community providers. Identify patients with potential ongoing health care needs and coordinate the establishment of appropriate individualised plans of care to meet the identified needs in the most effective manner. Contribute to the development, supervision, and training of all staff. Takes an active role in the planning and conduct of the nursing preceptership program. Peter Mac Nursing is represented and contributes to the development of evidence‑based position statement or papers, recommendations and guidelines where appropriate both nationally and internationally. Ensure adherence to organisational standards and mandatory training requirements. Adherence to legislation and organisational policy pertaining to privacy, confidentiality and the handling of personal information. Screening and Assessment Ensure referrals are triaged in a timely fashion. Ensure patients with complex needs are identified and supported in a timely manner to optimise patient outcomes. Referrals are triaged within 72 hours of receipt. Evidence based assessment tools are completed for new patients and referrals made as required. Demonstrates expertise in clinical assessment skills and care planning for patients with complex physical and psychosocial needs. Supports other nurses to develop expert clinical assessment skills and develop individualised care plans. Improving Quality and Safety of our services Ensure relevant education material for patients and other nurses is evidence based and up‑to‑date. Deliver evidence based nursing care where possible. Participate in and facilitate nursing research. Foster an environment that promotes education and research within the nursing division. Participate in continuing education sessions, committees, special projects and relevant professional groups. Educates the health care team regarding the concept and role of the Nurse Consultant. Collects and monitors relevant nurse‑sensitive KPI's for your stream. Demonstrates flexibility and creativity in identifying resources to meet patient care needs. Evidence of involvement in the development and / or recognition and / or evaluation of evidence‑based patient education resources. Evidence of delivery of high quality education. Evidence of participation in committees and relevant professional groups. Collection of nurse‑sensitive KPI data that can be used to inform practice change. Investigates appropriate procedures, reporting and documentation in the case of adverse events. Evidence of contributing to / being involved in at least one research / QA project per year. Evidence of submitting an abstract for conference presentation or manuscript for publication, minimum 1 submission every 2 years.
