About the Diabetes Education Service The Diabetes Education Service delivers comprehensive inpatient and outpatient diabetes education across Austin Hospital, Heidelberg Repatriation Hospital, and Royal Talbot Hospital, in close collaboration with our community partners. About the role As a Diabetes Clinical Nurse Consultant, you will play a pivotal role in providing expert support, education, and management for patients living with diabetes and their families. You will deliver high‑quality, evidence‑based care to optimise clinical outcomes and promote patient self‑management across both inpatient and outpatient settings. Reporting to the Manager, Diabetes Education, this position sits within the Medical & Cancer Services Division, and will ensure patients and their families receive comprehensive, timely, and effective diabetes education and support. Key responsibilities include: Providing prompt and efficient assessment and review of patients following referral. Identifying knowledge gaps among patients and staff, and developing targeted educational programmes to address these needs. Maintaining current knowledge and engaging in ongoing professional development in all aspects of diabetes education and management. About you You are a dedicated and compassionate nursing professional who demonstrates strong communication skills and a commitment to delivering excellence in patient care. You bring enthusiasm, initiative, and a genuine passion for advancing your expertise in diabetes education and management. You will also have: Extensive diabetes knowledge and clinical skills, with the ability to work independently as a sole practitioner. Demonstrated experience in diabetes education within an acute care setting. Proficiency with current diabetes technologies, including insulin pumps, continuous glucose monitoring (CGM), and flash glucose monitoring systems.
