PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community. PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services. The Viral Hepatitis Clinical Nurse Consultant (CNC) is a Registered Nurse who is responsible for providing enhanced models of care and clinical leadership in the diagnosis and management of people living with viral hepatitis. The CNC will provide clinical expertise and support viral hepatitis diagnosis and linkage to care across SESLHD Drug & Alcohol and primary care settings. The CNC will work closely with the HIV and Related Programs (HARP) Unit, Public Health Unit (PHU), and other district services who provide viral hepatitis care. The CNC will support SESLHD to achieve the NSW Health Viral Hepatitis Strategy targets, and the NSW Health Goal to eliminate Hepatitis C by 2028. Tertiary setting Proactive detection of viral hepatitis across SESLHD Drug & Alcohol sites, including electronic medical record (eMR) review to identify people with clinical indicators and/or risk factors for HCV. Support and provide nurse-led assessment and care planning for inpatients including linkage to primary care. Implement enhanced follow-up for HCV and HBV PHU notifications across the northern sector of SESLHD. Provide clinical expertise and support to implement SESLHD BBV Testing Policy (pending approval). Establish antenatal referral pathways for mothers living with HBV. Primary care setting Respond to PHU notifications by contacting general practitioners (GP) to support confirmatory HCV testing, GP remote consultation, treatment, and/or linkage to tertiary care. Respond to PHU notifications for HBV, including HBV assessment and care planning, contact tracing and management of contacts, and/or HBV education. Target and assist GPs in priority population areas to prescribe HBV and HCV treatment. Identify clients with complex co-morbidities requiring specialist care. Offer clinical support for practices engaging in viral hepatitis QI activities in partnership with CESPHN. Provide in-reach nurse-led shared care models in high case load practices (e.g. monitoring of people living with chronic hepatitis B). Community outreach setting Support roll out of Dried Blood Spot (DBS) and other testing modalities. Work with HARP Unit, Primary and Community Health (PaCH) and other partners to ensure targeted screening and linkage to care for at‑risk and hard to reach populations. The CNC will provide leadership and guidance to ensure excellence in clinical practice and optimal patient outcomes. The role requires data collection to evaluate progress. The CNC will be employed as Grade 2 according to the criteria outlined below, and in line with the NSW Public Health System Nurses’ and Midwives’ (State) Award. Qualifications and Experience Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia and current NSW Drivers License (if required by service). Minimum five years full time equivalent post qualification experience, with at least three years full time equivalent experience in the specialty field and post graduate qualifications relevant to the field of practice (viral hepatitis). Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing/midwifery practice. Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework. Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research. Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes. Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs. Demonstrated clinical expertise in client-centred consultancy with the ability to work independently.
