APS6 Complex Case Manager

A large federal government agency is seeking experienced clinical case managers to join its national Client Support Program. These roles support individuals and families presenting with complex mental health, physical health, and psychosocial needs, within a trauma-informed and recovery-oriented framework. About the program You will join a specialist client support function that provides time-limited, trauma-informed case management to individuals with complex health and wellbeing needs. The program focuses on capacity building, coordinated care and supporting clients to navigate government-funded services more independently. What you’ll be doing In this APS6-equivalent clinical role, you will: Manage a caseload of highly complex clients , acting as their primary point of contact. Conduct intake assessments , risk assessments and structured case reviews. Provide clinical judgment, triage and advice for clients with significant mental health or psychosocial issues. Escalate high-risk matters through senior clinicians and management. Coordinate services across internal programs, counselling services, ex-service organisations and community providers. Prepare clear, high-quality case notes, summaries and briefings. Build strong relationships across clinical and non-clinical stakeholders. This role is full-time at 37.5 hours per week . About you You thrive in complex environments and bring a strong clinical foundation to your case management practice. You are confident working with trauma, risk and multidisciplinary supports. Mandatory Tertiary qualification (Bachelor or higher) in Nursing, Psychology, Social Work, Occupational Therapy , or another recognised health discipline. Experience supporting clients with complex mental health, disability or psychosocial needs. Strong case management and assessment capability, including triage and telephone-based support. Excellent written and verbal communication. Commitment to trauma-informed, person-centred practice. Highly desirable Experience in Defence, veteran services, community mental health or similar environments. Knowledge of therapeutic approaches relevant to trauma and complex presentations. Compliance Ability to obtain a National Police Check . Ability to obtain and maintain a Baseline Security Clearance (can commence before clearance is finalised). Australian citizenship is required.
