APS6 Case Manager – Clinical

The Opportunity Canberra Consulting is partnering with a large Commonwealth agency that supports veterans and their families to access care, compensation and wellbeing services. We are seeking an APS6 Complex Case Manager – Clinical to join the Client Support Program within the Coordinated Client Support Branch. In this role you will deliver veteran-centric, trauma-informed case management to clients with complex needs, helping them navigate government systems and connect with appropriate clinical and psychosocial supports. This is a 3-month labour-hire engagement (37.5 hours per week) with the possibility of extension, based in one of several metropolitan and regional locations across Australia. Key Responsibilities Act as the primary point of contact for a caseload of clients with complex and multiple needs, providing trauma-informed, veteran-centric case management. Undertake intake, psychosocial and risk assessments, and develop case plans that align with the DVA Wellbeing Model and social determinants of health. Coordinate access to internal services (legislative entitlements, income support, rehabilitation and treatment) and external providers (health, mental health and community supports). Escalate high-risk presentations through established clinical and risk frameworks, including Case Review and Risk Assessment processes. Build and maintain strong working relationships with internal stakeholders, the ADF, ex-service organisations, community groups and service providers to streamline service access. Provide specialised clinical advice and coaching to colleagues to support consistent, high-quality case management practice and risk management. Prepare high-quality case documentation, client summaries and briefs, ensuring accurate records and timely updates in line with agency policies and procedures. Contribute to representational duties, including group presentations to internal and external stakeholders as required. Skills and Experience Tertiary qualifications (Bachelor’s degree or higher) in a relevant health field such as Nursing, Psychology, Occupational Therapy or Social Work, with current professional registration (AHPRA/AASW) where applicable. Demonstrated experience working with people who have complex support needs, mental health conditions and/or co-morbidities, using a trauma-informed, recovery-oriented approach. Strong case management experience, including intake, assessment, risk assessment, planning, coordination and review, ideally in health, mental health, disability or community services. High-level understanding of the impacts of disability, the social determinants of health and capacity-building models of support. Excellent written and verbal communication skills, with the ability to manage sensitive conversations, draft clear case notes and briefs, and liaise effectively with diverse stakeholders. Confidence using contemporary ICT systems (including Microsoft Office) and case management platforms. Demonstrated commitment to ongoing professional development. Eligibility and Security Australian citizenship is required. Ability to obtain and maintain a Baseline security clearance (sponsored by the client agency; clearance can be obtained after commencement).
