Job Summary We are excited to share an opportunity to join the West Ipswich Care Management initiative which supports high users or predicted high users of acute non‑elective beds. Care management is a way of intensively supporting this group of patients with proactive care coordination & additional GP and community matron capacity. Aims of the Service To improve the care of these complex patients Ease workload pressures across the system Reduce hospital admissions Key Responsibilities Attend a daily MDT meeting at a practice site to discuss the care plan for relevant patients. Carry out rapid response appointments and home visits as necessary. Provide proactive GP care planning for frailty, multimorbidity and other high‑need patients. Work collaboratively across organisational boundaries to deliver best care.
