We have positions in the Adult Care Coordination Team and Assertive Outreach Team which offer a collaborative and supportive service delivering recovery-oriented and strengths-based interventions to individuals living with severe and persistent mental illness. Adult Care Coordination The role is full-time, with hours from Monday to Friday, 8:30am to 5:00pm (subject to operational requirements) and involves managing a caseload, providing education and support to consumers and their families (between the ages 18-65), care planning with consumers, safety planning and crisis support, and coordinating care with the consumer and their key supports. Assertive Outreach Team Operating under a Flexible Assertive Community Treatment model, the team provides support across a 7-day rotating roster, offering varied shifts (8:30am–5:00pm, 10:00am–6:30pm, and 2:00pm–10:30pm) to meet the needs of consumers and carers. These positions are ideal for clinicians who are keen to learn and grow within a multidisciplinary environment. You’ll be part of a team that values person-centred care, professional development, and meaningful engagement with consumers and their support networks. Being a pool clinician is unique You will be filling a vacancy in one of the teams listed; as other vacancies arise throughout our service you may be required to share your skills with other teams in our service if future opportunities present at Liverpool, Ingleburn, Campbelltown, Ambarvale, Fairfield, Bowral and Wollondilly based mental health services. What You’ll be Doing The Mental Health Professional is responsible for providing evidence-based, strengths focused, recovery oriented assessment and treatment within a multi-disciplinary team and coordinating consumers’ recovery plans. The role is also responsible for non-clinical tasks including participation in quality improvement activities, clinical supervision and participation in administrative responsibilities as required. Where You’ll Be Working SWSLHD Community Mental Health Services provide comprehensive, community-based mental health care across South Western Sydney. These services cater to different age groups, including children, adolescents, adults, and older persons, ensuring specialised care throughout various life stages. The multidisciplinary teams offer a range of supports, including mental health assessments, crisis intervention, care coordination, and therapy. They work closely with inpatient services, GPs, and other community organisations to provide holistic care.
