You will be working in the community with older adults experiencing mental health issues who may be living in their own homes, with family, or in RACF. You will be providing recovery-oriented, person-centred biopsychosocial care planning and interventions reflected in all aspects of your practice and service delivery. You will have previous experience working with older adults in a health or community setting, including experience of assessment, care planning and interventions which are appropriate for and effective with older adults experiencing mental ill health. Previous experience in a care coordination role will be of benefit. Our teams work across the District and are based in either the Liverpool and Bankstown services (located within Liverpool and Bankstown Hospitals), or Macarthur/ Wingecarribee Services (Camden Hospital, Bowral Community Health Centre and at Queen St in the Campbelltown CBD). The current opportunity exists in either the Bankstown or Liverpool locations. Being a pool clinician is unique You will be filling a vacancy in the team listed; as other vacancies arise throughout our service you may be required to transfer into another position and share your skills within our service You will be based at Liverpool or Bankstown, however future opportunities may also present at Campbelltown, Ingleburn, Ambarvale, Fairfield, Bowral and Wollondilly based mental health services. What You'll be Doing The Mental Health Professional is responsible for providing evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team and coordinating consumers’ recovery plans. The role is also responsible for non-clinical tasks including participation in quality improvement activities, clinical supervision and participation in administrative responsibilities as required. Where You'll Be Working SWSLHD Community Mental Health Services provide comprehensive, community-based mental health care across South Western Sydney. These services cater to different age groups, including children, adolescents, adults, and older persons, ensuring specialised care throughout various life stages. The multidisciplinary teams offer a range of supports, including mental health assessments, crisis intervention, care coordination, and therapy. They work closely with inpatient services, GPs, and other community organisations to provide holistic care. Relevant qualifications as a Registered Nurse, Occupational Therapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW), or CSU Graduates: Bachelor of Health Science (Mental Health).
