The CNS2 working within the Neonatal Hospital in the Home (HITH) programme will deliver high‑quality nursing care to neonates and their families of the Royal Hospital for Women consistent with SESLHD and Newborn Care policies, procedures and standards. The role includes identification of suitable families to transition to the HITH model of care, pre‑discharge planning, parental education, a combination of home visiting and tele‑health, and finally discharge from the service. This will also include coordination and monitoring of service provision and education of staff members about the role and model of care. Selection criteria Current registration as a Registered Nurse and/or Registered Midwife with the Nursing & Midwifery Board of Australia (AHPRA) and current International Board‑Certified Lactation Consultant (IBCLC) qualification. Holding a relevant post‑registration qualification and at least 3 years’ experience working in the clinical area of the specified postgraduate qualification, or such other qualifications or equivalent experience deemed appropriate. Demonstrated ability to manage complex clinical care and service delivery issues and problem‑solve through the development and use of clinical management plans. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations. Demonstrated skills in the development and facilitation of patient and carer education. Demonstrated high‑level written and verbal communication, interpersonal skills, and demonstrated competence in the use of health‑related information technology. Detailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovation. Current unrestricted driver’s licence and willingness to travel.
