Key responsibilities: This Grade 1 role provides the opportunity to develop knowledge and skills in different areas of public health and is a part of the Social Work Casual Bank at Western Health. The role is supported by a structured supervision program focussing on reflective practice, ongoing professional development and learning. In addition to this, the position enables social work career progression through opportunities for further learning and participation in team and departmental activities. About you: To be successful in this role, you will: Provide a timely response to referrals, through application of an evidence based clinical assessment and intervention using a person-centred approach. Participate as an active member of the multidisciplinary team to enable safe discharge and facilitate patient flow. Participate in relevant committees, working groups and meetings as required. Ensure safe and high-quality patient care is delivered in line with Western Health values and policies. Support and contribute to research, quality and continuous improvement activities as required.
