Clinical Nurse Specialist (Community Palliative Care) - St Vincent's Public Hospital, Sydney NSW Health Darlinghurst, New South Wales, Australia Role title: Clinical Nurse Specialist (Grade 2) Role type: Permanent full time Department and location: Palliative Care Service, St Vincent’s Hospital, Darlinghurst Salary: $113,243.15 - $116,963.48 + super Better and fairer care. About the Role Coordinate, lead and participate in projects related to patient flow and care coordination from one service location to another Monitor patient wait times and estimated discharge dates through update and management of the electronic patient journey board and identify key "Waiting for What" (W4W) and patient exit blockers Lead multidisciplinary meetings in relation to patient flow and movement to ensure patients are seen in a timely manner, advising and negotiating with referrers and escalating any bed blocks Work in a dynamic team leading our approach to the provision of quality care to community clients of the Palliative Care Service Provide evidence‑based nursing care to people of all ages and cultural groups, including individuals, families and communities Take an extended leadership role in the coordination of nursing and health care, providing expert nursing assessment and triage to facilitate optimal health outcomes; feed back to staff on how to improve patient satisfaction or efficiency About You Registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency Current NSW driver's licence (Class C or above) Relevant post‑registration qualifications and at least three years’ experience working in palliative care Demonstrated ability to work within the interdisciplinary team at a senior level with a commitment to ongoing professional development and effective communication and interpersonal skills Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in daily practice as a registered nurse with effective time management skills and ability to prioritise competing workloads Benefits Join us on our mission to provide care to all, including the most at‑risk people in our community Discounted access to many gyms for you and your family with Fitness Passport Access to discounted private health insurance with Medibank Access to a government salary sacrifice program; make a proportion of your salary tax‑free Committed to financial support for educational development, scholarship programs, paid study leave About the Hospital In this role you will be based at St Vincent’s Hospital, Darlinghurst A busy inner‑city hospital in a beautiful location Surrounded by great cafes, galleries and theatres, parks, nightlife, and nearby glorious beaches A place of expert clinical care for at‑risk people Closely partnered with universities and affiliated research institutes, many of which are located on‑campus Centres of excellence include heart & lung transplantation, bone marrow transplantation, cardiology, cancer care, AIDS/HIV, respiratory medicine, mental health, homeless health, drug & alcohol services, and correctional health Visa & Work Rights Please note, visa sponsorship is not available for this position.
