The Rev. Bill Crews Foundation (‘BCF’) is one of Sydney’s most trusted frontline charities, supporting the community for nearly 40 years. Our mission is to address homelessness and hardship by providing essential services including food, social welfare, healthcare, education and community programs. Our Social Health and Wellbeing (SHW) team is often the first point of contact for individuals experiencing complex and multiple challenges. The team provides a broad range of assistance, from crisis intervention and short-term case management to support with essentials such as bills, medical, dental and complementary healthcare services. THE ROLE This position is central to welcoming individuals and families seeking support. As an Intake Case Worker, you will assess presenting needs, provide short-term case management and link clients with appropriate internal and external supports to promote safety, stability and wellbeing. This role will be based within our newly established Blacktown Medical Clinic, providing an opportunity to contribute to a growing service that supports some of Western Sydney’s most vulnerable community members. The role will commence after the clinic opens on 5 January 2026 . The role offers flexibility of full-time or part-time variety and the opportunity to work within a values-driven team committed to making a genuine difference. WHAT YOU'LL DO Reporting to the Client Services Coordinator, you will: Be the first point of contact for new and returning clients, conducting intake assessments and identifying immediate and longer-term needs. Deliver short-term, solution-focused case management and connect clients with internal supports including meals, healthcare, dental and social programs. Facilitate referrals to external agencies including housing, mental health and AOD services. Maintain up-to-date knowledge of local service networks to ensure clients can access timely and appropriate assistance. Keep accurate and confidential client records and contribute to required reporting. Work collaboratively with our dedicated staff and volunteers across all frontline programs. Participate in debriefing, supervision and ongoing professional development to support safe and sustainable practice. WE'RE LOOKING FOR You are a compassionate and proactive practitioner who is comfortable working in a dynamic, client-focused environment. You can build rapport quickly and provide practical, person-centred support. You’ll also bring: Tertiary qualification in social work, community services, social science, welfare or a related field. Minimum 2 years’ experience working with vulnerable individuals, ideally within housing, mental health or community services. Understanding of social housing systems and referral pathways. Experience supporting clients with mental health and alcohol or other drug (‘AOD’) needs. Ability to coordinate with external service providers for assessment, support and ongoing case management. Proficiency in client management systems such as Penelope or SharePoint with sound documentation skills.
