About the School Ashburton Drive Primary School (PS) is proud to be part of the Connected Community School trial, a four‑year Department of Education initiative that aims to connect students, families and the school community with services and supports to improve education and wellbeing outcomes. Role Overview We are looking for an experienced and highly skilled Social Worker to join our Connected Community School trial team and work collaboratively with community services, other government agencies, health providers and local organisations. The role will build positive, trusting relationships and better connect students and their families with the supports they need. Full details of the role, selection criteria and training requirements are listed in the attached Job Description Form (JDF). Responsibilities Build rapport with the school community. Assist in a whole‑school approach that strengthens partnerships, builds positive community relationships and supports students and families to improve wellbeing, learning and life outcomes. Co‑operate with community services, government agencies, health providers and local organisations to coordinate support for students and families. Conduct assessments of referred students, identify suitable services and supports, prepare plans and engage in social work intervention, including interviewing family members and making home visits. Participate in multidisciplinary team meetings; consult and advise other staff regarding specific cases. Maintain accurate records, case notes, data and reports that support evidence‑informed practice and contribute to trial evaluation. Qualifications & Experience Clear understanding of contemporary professional and practice issues, legislation, and policy relating to Social Work. Demonstrated knowledge of working with children and families who are at high risk. Eligibility Australian or New Zealand citizen, permanent resident or holder of a valid visa with work rights for the duration of the appointment. Valid Working With Children Check before starting. Consent to a Nationally Coordinated Criminal History Check and obtain a current Screening Clearance Number issued by the Department of Education’s Screening Unit before starting. Additional eligibility and training requirements are outlined in the attached Job Description Form (JDF). Demonstrated strong conceptual and analytical skills with the ability to identify and clarify issues and problems and generate strategies to address them. Demonstrated knowledge of school systems and operations and skills in working with a multidisciplinary team setting. Demonstrated skills and experience working with children and families.
