Quality Progressive Care Pty Ltd – Deer Park VIC NDIS SUPPORT COORDINATOR Quality Progressive Care (QPCare) POSITION PURPOSE The Support Coordinator plays a vital role in empowering NDIS participants and their families to navigate the NDIS with confidence. The purpose of this role is to strengthen participants' capacity, ensure they can access the right supports at the right time, and assist them to work toward meaningful life goals. At QPCare, the Support Coordinator is responsible for building strong, collaborative, and trust-based relationships with participants, supporting them to implement their NDIS plans effectively, and ensuring they receive high-quality, person-centred care that promotes independence, stability, and improved quality of life. PERFORMANCE OBJECTIVES The primary objective of the Support Coordinator is to support participants to understand, activate and implement their NDIS plan while fostering greater community participation, resilience, and self-advocacy. This includes identifying appropriate supports, coordinating service delivery, monitoring progress, and ensuring participants are able to maximise the value of their NDIS funding. A QPCare Support Coordinator will consistently demonstrate professionalism, compassion, cultural awareness, and strong advocacy to ensure participants achieve the best possible outcomes. KEY RESPONSIBILITIES Participant Relationship & Support Meet with participants regularly (in person, via phone or online) to discuss their needs, goals, and priorities. Build strong, culturally sensitive relationships with participants, families, and informal supports. Maintain accurate, up-to-date case notes and documentation for each interaction. Coordination of Supports Connect participants with appropriate services, including allied health, community supports, psychosocial supports, and mainstream services. Coordinate multidisciplinary support teams to ensure timely and consistent service delivery. Liaise with providers to ensure they understand participant needs, preferences, and goals. Education & Navigation Provide clear education to participants and families on the NDIS, funding categories, budgets, Service Agreements, and Provider Service Bookings. Support participants to understand plan expenditure, the NDIS Pricing Arrangements, and value-for-money principles. Monitoring, Review & Reporting Monitor the progress and outcomes of participants and adjust support plans as required. Complete individual planning documents within 2 weeks of a service agreement being signed. Update planning documents annually or sooner if circumstances change. Conduct vulnerability check-ins and elevate risks appropriately. Prepare reports for NDIA plan reviews, including justification of support coordination outcomes. Advocacy & Capacity Building Advocate for participants to ensure they receive appropriate and high-quality services. Work collaboratively with informal supports to strengthen the participant’s network. Support participants to build skills, independence, and decision-making capacity. Assist participants to manage crises and navigate complex situations with clarity and direction. Documentation & Compliance Maintain accurate, timely, and high-quality record-keeping for all participants. Monitor budgets and manage funding to ensure responsible expenditure. Complete all recommended quotes, referrals, and assessments. Report weekly to the Director on participant progress, challenges, and service coordination activities. Follow all QPCare policies, procedures, and the NDIS Code of Conduct. Manage conflicts of interest in accordance with QPCare’s conflict of interest guidelines. Technical Support Support participants to: Understand the NDIS plan structure, budgets, and support categories Navigate myGov and the myplace portal Interpret Service Agreements Set up Service Bookings Resolve crises through problem-solving and coordination Connect participants with advocacy organisations when required. Networking & Community Engagement Develop and maintain strong networks with local providers, hospitals, community organisations, and allied health professionals to facilitate seamless referrals. Stay informed about local services, eligibility criteria, and emerging supports that benefit participants. SCREENING REQUIREMENTS 100 points of identification Proof of right to work in Australia NDIS Worker Screening Check International Police Check (if applicable) Working With Children Check (WWCC) Full License & Vehicle (Will be required to drive out to Participants when required) KEY QUALIFICATIONS & SKILLS Completion of the NDIS Worker Orientation Module Infection Control Training Certificate Tertiary qualifications in disability, mental health, social work, aged care, or related fields (preferred) Minimum 3-5 years’ experience in Support Coordination or case management Strong knowledge of: NDIS Pricing Arrangements and Limits NDIS Practice Standards and Quality Indicators NDIS rules, funding categories, and reporting expectations Excellent communication and interpersonal skills Strong community and local provider knowledge Ability to work autonomously and manage competing priorities High level of professionalism, cultural competence, empathy, and problem-solving Strong documentation and time-management skills #J-18808-Ljbffr
