Establishedin 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlledorganisation committed to providing culturally appropriate,comprehensive primary health care services and community development within theBiluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region inthe Northern Territory. ABOUT THE ROLE The SEWB Support Worker will work inpartnership with their multidisciplinary team to support youth and families intheir social and emotional well-being. This role is integral to our commitmentto delivering holistic health services and provide culturally safe and traumainformed support that promotes wellbeing and links youth with the SEWB supportthey need. The SEWB Support worker is expected to provide information,education, advocacy, assessments, and brief interventions to encourage andsupport client's wellbeing. The SEWB support worker will also facilitate groupsessions that empower and support community and promote mental health. WHAT YOU WILL DO Maintainregular contact with clients within the scope of practice who have beenreferred by clinics, other providers, or self-referral to SEWB. Workwith a strengths-based approach to develop, implement, and monitor a casemanagement plan to address specific client needs in collaboration with the SEWBmulti-disciplinary team. Provideintervention and support to clients who are at risk of psychosocial distress orexperiencing low acuity concerns. Within the worker's expertise and skills,identify immediate risks and needs, explore possible interventions, develop andimplement support plans, and facilitate referrals. Otherduties as required, consistent with skills and experience, as directed by the relevant member of DDHS SELECTION CRITERIA Essential Identify as Aboriginal Demonstratedexperience working with Indigenous people and the ability to communicateeffectively and in a culturally appropriately manner Demonstratedknowledge of current issues, standards and trends in the delivery of mentalhealth and social and emotional wellbeing services to Aboriginal people; Demonstratedawareness of and sensitivity to Aboriginal culture and history and knowledge ofissues impacting on Aboriginal children and families, youth and adults andthose affected by Stolen Generations' policies and practices; Demonstratedability to work within and maintain strict confidentiality guidelines andpolicies; Demonstratedcommunication and interpersonal skills with the ability to liaise with peoplefrom a wide range of cultural and social backgrounds to both obtain and conveyinformation relevant to their care; Demonstratedability to interact effectively with clients, carers and other professionalsfrom diverse cultures; Abilityto work independently, and as part of a team, with an ability to plan, manageand prioritise tasks and resources to deliver outcomes within time and budgetconsiderations. Proficiencyin and commitment to the use of electronic information systems for themaintenance of clinical and service delivery records. Desirable Experience working in a community servicesprogram or community education program. Certificate qualifications in Community Servicesor equivalent IN RETURN WE OFFER 6 weeks annual leave. 5 days paid leave duringend-of-year shut down (additional to annual leave).
