The role assists in the clinical management of patients with CKD in conjunction with the Nursing team, Allied Health Team, Medical team and the referring Nephrologist. This position requires care to be delivered across renal services in inpatient, outpatient, and community health settings. This position will work in collaboration with the Director of Renal Medicine, Nephrologists, Renal Nurse Manager, Nursing teams, allied health and community care providers to identify, conduct, facilitate and promote best patient outcomes. Regular liaison will occur with the Renal Service Director, Renal Nurse Manager, relevant Nurse Unit Manager’s and Clinical Nurse Educator’s in each department to identify staff educational needs and develop strategies for the education of staff, consumer groups, patients, and others. The position will promote a service which fosters clinical excellence, research and professional leadership and contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards, and evaluating their outcomes. This role is responsible for providing services which facilitate effective management of the care in the given specialty. Responsible to: Renal Nurse Manager SWSLHD Director of Renal Service SWSLHD Strategically linked to the Cardiovascular Stream through the Renal Service What You'll be Doing The Nurse Practitioner/Transitional Nurse Practitioner is educated and endorsed to function autonomously and collaboratively in an advanced and extended clinical role. The NP/TNP role includes assessment and management using nursing knowledge and skills. The role may include, but is not limited to, the direct referral of patients to other health care professionals, prescribing medications and initiating and interpreting diagnostic investigations. Where You'll Be Working Liverpool Hospital, founded in 1813, is the major health service for south-western Sydney, providing services to the local government area of Liverpool City Council as well as district services to residents and visitors in the area. It also provides a range of state-wide services such as critical care and trauma, neonatal intensive care and brain injury rehabilitation. The Hospital provides a health service of international standing, with 23 operating theatres, capacity for 877 beds, diagnostic and imaging services, emergency and trauma care, maternity, paediatric, cancer care, mental health, ambulatory care, allied health and medical and surgical services from birth to aged care. Liverpool Hospital is currently undergoing a major redevelopment. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) As a Transitional Nurse Practitioner you must have 5 years full-time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. An approved postgraduate nursing/midwifery qualifications relevant to the field in which the employee is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organization. As a Nurse Practitioner you must Hold a Master’s Degree meeting eligibility for endorsement as a Nurse Practitioner from the Nursing and Midwifery Board of Australia. Be authorised by the NMBA in accordance with s95 of the Health Practitioner Regulation National Law (NSW) No 86a to use the protected title of ‘Nurse Practitioner’ and therefore utilise the extensions to clinical practice including prescribing, referral, diagnosis, initiation of diagnostics and discharge from care. Demonstrated ability to undertake comprehensive health assessments of patients within target populations in order to collect relevant clinical data to inform clinical and diagnostic decision making processes. Demonstrated ability to articulate sound reasoning behind clinical, diagnostic and prescribing decisions that are informed by evidence based practice and made within a nursing context. Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs. Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise. Demonstrated evidence of the use of critical and reflective thinking, complex reasoning and analysis to guide assessments, clinical judgements and decisions whilst managing complete episodes of patient care.
