An exciting opportunity exists for a clinician with a strong interest in and commitment to Mental Health to join the experienced and dedicated Acute Community Mental Health Team at Ryde Community Mental Health Centre. What you'll be doing The Ryde Acute Team model of care provides a combination of short-term and medium to longer-term support in the form of a multidisciplinary team approach to collaborative care, including: Mental Health triage and assessments for consumers within the community experiencing acute mental health problems. Mental health assessments in Ryde ED after hours. Short term mental health treatment in the community often as an alternative to hospital, including care coordination and longer-term case management as clinically indicated. Collaborative care-planning for consumers transferring between hospital and community, follow up support and onward referrals where appropriate. Brief therapeutic interventions such as BIC based on Project Air model for Personality Disorders. Working in a multidisciplinary team across a 7-day rotating roster with morning and afternoon shifts. Requirements Current AHPRA registration for relevant discipline - Psychology, Occupational Therapist, Registered Nurse, Eligibility for membership with AASW for Social Workers. Extensive case management/care co-ordination skills. Demonstrated ability to work within a multi-disciplinary team setting. Current NSW Driver's licence. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply. This is a NSW Health Category A position which requires immunisation and screening for certain diseases.
