About The Role The Planning and Referral Team (PaRT) are experienced health clinicians who work with the treating teams, patients, families and services in planning an appropriate, timely and safe discharge. PaRT are responsible for the facilitation and coordination of patient transfers to bed-based services and the coordination of referrals to community supports. A member of the PaRT team will be able to assist with discharge planning in collaboration with the medical and allied health teams. They will be involved in making required referrals, information sharing and communication.
