Our Team: Join our dynamic, supportive and flexible team to provide clinical and preventative health services to local Aboriginal communities in NSW and the ACT. The Aboriginal Health Team is multi-disciplinary team of Enrolled and Registered Nurses, Aboriginal Health Workers, Aboriginal Outreach Workers and Care Coordinators, supported by a variety of Specialists and Allied Health Workers. The GPH Aboriginal Health Team is dedicated to enhancing the health and wellbeing of Aboriginal and Torres Strait Islander communities through a holistic approach based around four key pillars. We prioritise genuine and culturally respectful engagement with local Aboriginal communities, ensuring their voices and needs guide our initiatives. Our commitment extends to advocating for engagement with health screening and service access, the delivery of effective care coordination, and chronic disease management, alongside proactive health promotion strategies. Our Aboriginal Outreach Worker will share our passion for fostering a healthier future and strengthening connections within these communities. GPH values a diverse workforce, this is an Aboriginal Identified position authorised under S14d of the Anti-Discrimination Act 1977 (NSW) About the position: We are currently looking for an Aboriginal Outreach Worker to join our team. This role is vital in supporting the delivery of care coordination and health services to local Aboriginal and Torres Strait Islander communities in the Northern Sydney area, covering the Northern Beaches, Hornsby, and Ryde regions, with a focus on improving access to healthcare, health screening, chronic disease management, and health promotion. As an Aboriginal Outreach Worker, you will be part of a multidisciplinary team dedicated to promoting community engagement and health promotion activities. You will help individuals and families attend health appointments and clinics, contributing to their overall wellbeing and that of the community. This position will be based in Macquarie Park, and may include working from additional locations including Dee Why. This opportunity is available part-time (30 hours per week), fixed-term to 30 June 2026, with the possibility of extension. You will: Identify as an Aboriginal and / or Torres Strait Islander and be an active member within the local Aboriginal communities Certificate III or IV in Aboriginal Health Work or health related discipline or willingness to work towards Demonstrated understanding of issues affecting Aboriginal people and their health Demonstrated ability to effectively engage and communicate sensitively with Aboriginal people to address their health needs Why join GPH?
