Care Facilitator

About the role Casual opportunities available with a mix of AMs, PMs and Nightduty shifts, Monday to Friday Based in Byford Join a compassionate team and make a meaningful difference in the lives of our aged care residents. This position is based at our 68‑bed facility in Byford, where you'll work closely with a multidisciplinary team and other stakeholders to deliver person‑centred care to residents. You'll play a key role in supporting residents' physical and emotional wellbeing, helping them achieve their personal goals and enjoy meaningful daily activities. Your responsibilities will include promoting independence, coordinating with staff and nursing teams, and assisting with everyday tasks such as personal care, mobility, meals, household duties, and leisure activities. With strong communication and interpersonal skills, both written and verbal, you're able to build respectful relationships with residents, families, and colleagues. Ideally, you hold a Certificate III in Individual Support (or similar), or you are working towards it—or willing to obtain it.
