Full time position based at BlackburnMaximum term contract until 30 June 2025Provide information, advice and individual support to carersYour new roleOur Carer Services Program delivers a range of support services to carers who provide unpaid care and support to a family member or friend, who is frail due to age or has dementia, a disability, mental illness, terminal illness or chronic health condition.As an Assessment and Planning Officer, you will utilise the Carers StarTM conversation tool to understand and assess the specific needs associated with the caring role and develop goal orientated care plans in conjunction with carers.
