Employment type: Temporary Full Time (Up to 18/12/2026) Position classification: Physiotherapist Level 1 / 2 Remuneration: $37.03 – $54.94 per hour Hours per week: 38 Location: Blacktown, Mount Druitt, and Parramatta Community Health Centres Where you’ll be working You’ll join the Chronic & Complex (CAC) multidisciplinary teams across Blacktown, Mount Druitt and Parramatta Community Health Centres, part of Western Sydney Local Health District (WSLHD). Our service is dedicated to supporting older people to maintain independence, wellbeing and community participation. You’ll be part of a supportive and experienced allied health environment that values collaboration, growth and quality care. Work across well‑resourced Community Health Centres within WSLHD Join a welcoming, multidisciplinary team committed to person‑centred care Support clients to live independently and improve functional ability Be part of a culture focused on innovation, respect and continuous learning Access strong professional development, supervision and training programs What you’ll be doing As a Chronic & Complex Physiotherapist, you will deliver high‑quality, evidence‑based physiotherapy interventions to support older adults living with chronic, complex and age‑related conditions. You’ll work independently and collaboratively to assess needs, set goals, and implement tailored treatment plans that enhance mobility, safety and quality of life. Complete comprehensive assessments, including falls risk and functional screening Deliver physiotherapy interventions in client homes, centres and via telehealth Develop and review care plans in partnership with clients, carers and the multidisciplinary team Prioritise and manage caseloads aligned to clinical needs and the CAC Model of Care Participate in quality improvement, research, supervision and ongoing professional development How to apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Essential criteria Qualification: Tertiary qualifications in physiotherapy Registration: Current general physiotherapy registration under AHPRA Selection criteria Demonstrated experience and skills in physiotherapy assessment, planning and evaluating interventions for clients with chronic and complex health conditions. Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community. Demonstrated ability in the use of relevant evidence‑based, best practice guidelines to guide decision making for best client outcomes. High‑level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases. Ability to work independently and as part of a multidisciplinary team. Current driver’s licence valid within Australia.
