Chronic & Complex Physiotherapist – Community Health Employment Type: Temporary Full Time (Up to 18/12/2026) Position Classification: Physiotherapist Level 1/2 Hours Per Week: 38 Location: Blacktown, Mount Druitt and Parramatta Community Health Centres Remuneration: $37.03 – $54.94 per hour What You’ll Be Doing Complete comprehensive assessments, including falls risk and functional screening Deliver Physiotherapy interventions in client homes, centres and via telehealth Develop and review care plans in partnership with clients, carers and the multidisciplinary team Prioritise and manage caseloads aligned to clinical needs and the CAC Model of Care Participate in quality improvement, research, supervision and ongoing professional development Essential Criteria Tertiary qualifications in Physiotherapy Current general Physiotherapy registration under AHPRA Selection Criteria Demonstrated experience and skills in Physiotherapy assessment, planning and evaluating interventions for clients with chronic and complex health conditions Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community Demonstrated ability in the use of relevant evidence based, best practice guidelines to guide decision making for best client outcomes High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases Ability to work independently and as part of multidisciplinary team Current Drivers Licence valid within Australia How To Apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date.
