Community-focused service delivering integrated chronic and complex care Supportive multidisciplinary team with strong clinical collaboration Culture of learning, innovation and continuous quality improvement Opportunities for supervision, mentoring and professional development A welcoming environment dedicated to holistic, person-centred care What you’ll be doing As the Chronic & Complex Social Worker, you will apply advanced clinical skills to assess, support and empower clients facing complex medical, social and psychological challenges. Working both independently and collaboratively, you will deliver high-quality social work interventions that enhance client outcomes and strengthen support networks. Conduct comprehensive assessments and deliver evidence-based interventions Work autonomously while seeking specialist advice when cases require escalation Provide education and guidance to clients, carers and referring professionals Liaise closely with the multidisciplinary team to coordinate care and monitor progress Participate in supervision, continuing education, and student clinical placements How to Apply Apply online by submitting your resume and addressing the criteria listed below by providing examples that demonstrate your ability before the closing date. Essential Criteria Qualifications: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW) Selection Criteria Demonstrated post graduate clinical experience working with clients with chronic and complex Demonstrated clinical experience or understanding of the needs of clients accessing the health care system. Demonstrated clinical experience or understanding of the needs of clients with chronic and complex health needs. Well-developed written, verbal and non-verbal communication skills, effective time management and ability to prioritise workload. Able to work as an effective team member as well as work independently, including flexibility and adaptability. Good computer skills, including experience using Cerner eMR or equivalent. Demonstrated understanding and application of quality improvement principles. Drivers license valid within Australia and willingness to use for work purposes.
