Social Worker Level 1/2 – Illawarra Transitional Aged Care Service NSW Health is looking for a Social Worker to join our team. Applications close: Sunday 30 November 2025 Salary: $76,364 - $108,943 per annum + 12% super & salary packaging. Permanent Full-Time (Part-Time considered) . Port Kembla Hospital (PKH) . REQ Be pivotal to the coordination of patient care to ensure optimal health outcomes for elderly clients in the community in collaboration with a multidisciplinary team. The social worker will maintain a case load including case management responsibilities for a number of clients from admission to discharge. The role includes comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counselling, liaison with community agencies and relevant parties including medical teams. Support often extends to client’s families and carers. Working in the community setting requires a level of autonomy in problem-solving without direct clinical supervision when completing in-home assessments and interventions. Ideally, this position would suit someone with at least 2 years’ experience. The position functions as part of an interdisciplinary team with a teamwork approach to the provision of home-based intervention to frail and often socially complex older clients who would like to return home but are unable to do so without additional care and support. As a part of the new Shellharbour Hospital and Integrated Services Project, a new Community Health Centre is being built on the old Port Kembla Hospital site. Located on Dharawal Country, Warrawong Community Health Centre will bring together a range of services including community mental health, drug and alcohol, community nursing, and child and family services. Until the new Community Centre is complete, some allied health services will continue to operate out of the Port Kembla Hospital site. Benefits Of Working With ISLHD.
