ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes, following discharge from hospital, as well as assisting with the decision-making process around long term care planning. The social worker will maintain a case load including case management responsibilities for a number of clients from admission to discharge. The role includes comprehensive psycho-social assessments, care planning, risk assessment, education, advocacy, counselling, liaison with community agencies and relevant parties including medical teams. Support often extends to client’s families and/or carers. Working in the community setting requires a level of autonomy in problem-solving without direct clinical supervision when completing in‑home assessments and interventions. Ideally, this position would suit someone with at least 2 years’ experience. The position functions as part of an interdisciplinary team with a teamwork approach to the provision of home‑based intervention to frail and often socially complex older clients who would like to return home but are unable to do so without additional care and support.
